The label reads Spindle Amplifier Module, A06B-6111-H011#H550, revision B, rated input 283-339V DC 13.2 kW, maximum output voltage 240V AC, rated output current 48A, manual no. B-65282, standard EN50178 and UL508C, serial V05331384, with CE and UL recognized marks and Fanuc Ltd, Yamanashi 401-0597, Japan. The label does not show the series name. That is from the Fanuc datasheet.

A -SP13 tag is on the front. The front panel has a two digit ALM/ERR status display and CXA2B, CXA2A, JX4, JY1, JA7B, JA7A, JYA2, JYA3 and JYA4 connector labels. The lower cover is marked U, V, W and ground.
