The nameplate reads assembly 805405-3R, manual S-3060, W/D 30332, input 230-460 VAC at 15 A and 50/60 Hz, output 500 VDC at 15 A, and PC board 0-60014-C. It is marked Twinsburg, Ohio, U.S.A. and carries a barcode reading ST0YX0TV.

The black housing has scratches and scuffs on the cover. The front carries the AutoMax Distributed Power System Field Power Module label and the Reliance Electric logo. A ground symbol is visible at the terminal end. The unit was packed in bubble wrap and packing peanuts in a plain shipping box.
