Manufactured by Square D Company and made in the USA, it carries Form S, Series A designation. NEMA Size 4 corresponds to a polyphase horsepower rating of 40 HP at 200 VAC, 50 HP at 230 VAC, 75 HP at 380 VAC, and 100 HP at 460/575 VAC per the nameplate polyphase rating table.
