The nameplate reads VFS11-4022PL-WN (5), 2.2 kW, 4.2 kVA, 3 HP, input 50/60 Hz at 8.9/6.7 A, output 3-phase 380/500 V at 0.5/500 Hz and 5.5 A, S.Ckt 5000 A, fuse CC/J 15 A max, serial number 1740 01021408 0027, lot number 07J 101MY80, Made in Japan, Motor Protective Device Class 10. The input voltage line is scratched on the nameplate. The front label reads VF-S11, 3PH-380/500V-2.2kW/3HP.

The front has a Toshiba keypad with RUN, STOP, MODE and ENT buttons and a dial. Wires are attached at the terminal end, and the aluminum heatsink base has mounting feet.

Sold as surplus, untested. This unit has not been powered on or tested by MRO Exchange, and the keypad, display and output have not been verified.
