The front label reads VF-S11, 3PH-380/500V-2.2kW/3HP.

The front has a Toshiba keypad with RUN, STOP, MODE and ENT buttons and a dial. Wires are attached at the terminal end, and the aluminum heatsink base has mounting feet.
